Overview of Infertility


  • Infertility is a disease of the male or female reproductive system.

  • Infertility is defined as the failure to achieve a pregnancy after 12 months or more of regular, unprotected sexual intercourse.

  • Infertility may occur due to male, female, or unexplained factors. Some causes of infertility are preventable.

  • Infertility affects millions of people. Estimates suggest that approximately one in every six people of reproductive age worldwide experience infertility in their lifetime.

  • Male infertility is most commonly caused by problems in the ejection of semen, absence or low levels of sperm, or abnormal shape (morphology) and movement (motility) of the sperm.

  • A range of abnormalities of the ovaries, uterus, fallopian tubes, and the endocrine system, among others, may cause female infertility.

  • You can have primary or secondary infertility. We refer to someone who has never been pregnant as having primary infertility. We refer to someone with at least one pregnancy as having secondary infertility.


Why does infertility occur?


Many issues with the male or female reproductive systems can lead to infertility. However, there are situations where the causes of infertility cannot be explained.

Infertility in the female reproductive system can result from:

  • Ovarian disorders like

    • polycystic ovarian syndrome and

    • other follicular disorders;

  • Endocrine system disorders that result in imbalances of reproductive hormones;

  • Tubal disorders like

    • blocked fallopian tubes, which are in turn caused by untreated sexually transmitted infections (STIs), complications of unsafe abortion, postpartum sepsis, or abdominal/pelvic surgery; and

  • Uterine disorders can be

    • inflammatory (like endometriosis),

    • congenital (like septate uterus), or

    • benign (like fibroids).

  • The hypothalamus and pituitary glands are part of the endocrine system. Common conditions that impact this system include hypopituitarism and pituitary tumors.

Infertility in the male reproductive system may arise from:

  • Obstruction of the reproductive tract results in dysfunctions in semen ejection. Blockages may arise in the ducts responsible for transporting semen, including the ejaculatory ducts and seminal vesicles.

  • Blockages frequently result from injuries or infections of the genital tract and hormonal disorders that cause abnormalities in hormones produced by the pituitary gland, hypothalamus, and testicles.

  • Hormones like testosterone play a crucial role in regulating sperm production. Disorders that lead to hormonal imbalance include pituitary and testicular cancers.

  • Testicular failure to produce sperm may occur due to conditions such as varicoceles or medical treatments that adversely affect sperm-producing cells, including chemotherapy, as well as abnormalities in sperm function and quality.

  • Abnormalities in sperm morphology and motility adversely impact fertility. The use of anabolic steroids can lead to abnormal semen parameters, including sperm count and morphology (1).

  • Lifestyle factors, including smoking, excessive alcohol consumption, and obesity, can impact fertility. Furthermore, exposure to environmental pollutants and toxins can adversely affect gametes (eggs and sperm), reducing quantities and diminishing quality.
