Recognized in Ayurveda asthis procedure purifies the body by removing vitiated Rakta at local site. PURVAKARMA: Preparatory Phase Checklist Systemic Pre-treatment Snehana (Oleation therapy) - Day before procedure Swedana (Sudation/Fomentation) - Mobilizes doshas Body rendered soft for dosha disintegration Vitiated humors mobilized toward periphery Local Site Preparation Clean site with sterile, plain water ONLY Avoid soap, disinfectants, antiseptics, alcohol Patient in stable, comfortable position (supine/sitting) Anatomical site fully exposed for duration Jalauka (Leech) Maintenance House in clean earthen pots with dechlorinated water Change water every 2-6 days for cleanliness Housing density: ~2 leeches per 250 ml water Select only Nirvisha Jalaukas (non-poisonous) Must exhibit active movement when selected Purification & Activation Place leech in turmeric water (Haridra) Smear with turmeric-mustard paste Duration: 1 Muhurta (30-48 minutes) Transfer to fresh water; observe vigorous movement Purpose: Disinfect, alleviate exhaustion, stimulate appetite Optimal Timing & Season Morning hours preferred (leeches fresh, active; patient at rest) • Classical texts advise during Sharada Kala (Autumn) when Pitta Dosha is naturally prone to vitiation II. PRADHANAKARMA: Main Procedure Checklist 🎯 Execution & Leech Application Place leech mouth precisely over target site Use paper/cardboard with hole for small lesions Hold leech's tail gently until attachment begins If leech refuses to attach:  Prick skin with sterile needle for blood drop Apply milk, honey, or ghee to skin surface 👁️ Monitoring & Maintenance Sucking phase: 30-70 minutes typically Cover leech lightly with moist cotton wool Apply water drops to keep leech moist throughout Blood removal: ~5 mL per leech per session Signs of Effective Sucking: • Central body distention • Visible pulsations Clinical Endpoint: Shuddha Rakta (Pure Blood) Patient Feedback Signs Itching and/or pricking/burning sensation Therapeutic Principle Stop when pure blood flows; prevent depletion Detachment Method Turmeric (Haridra) or Rock Salt (Saindhava) III. PASCHAT KARMA: Post-Therapy Management Patient Wound Care Allow slight bleeding (few minutes) Cleanse with Triphala or saline Apply healing agents: Haridra (Turmeric powder) Satadhauta Ghritha (Medicated ghee) Nimb-Haridra oil for complex wounds Light bandage for 6-12 hours Promotes Vrana Ropan (tissue healing) Leech Detoxification (Vamana) Critical: Must purge ingested blood Sprinkle rice flour near mouth Smear oil + rock salt (Saindhava) Massage gently tail-to-mouth Or: Keep in turmeric water for purging Rehabilitation: Saline → Turmeric water → Fresh water Rest minimum 7 days before re-use Samsarjana Karma (Diet) Graduated Dietary Protocol: 1. Initial Phase:  Liquid-dominant foods (thin gruel, porridge) 2. Transitional Phase:  Gradually heavier foods with mild spices 3. Full Recovery: Normal diet resumed slowly Purpose: Rekindle Agni Prevents Ama formation from weakened digestion ⚠️ Lifestyle Restrictions (Parheza) Rest: Physical (Kayika) and mental (Manasika) rest mandatory                                     Hygiene: Only warm water for drinking and bathing Avoid: Strenuous activity, sun exposure, wind, heavy travel Intimate Activity: Strongly advised against during recovery ABSOLUTE CONTRAINDICATIONS     : Hemophilia • Leukemia • Severe Anemia • Hypotonia • Anticoagulant Therapy • Pregnancy/Lactation • Very Young/Very Old/Debilitated
