"Pancha" means five and "karma" means action. The system is described in the Charaka Samhita and Sushruta Samhita, the foundational texts of Ayurvedic Medicine.

1. Vamana

Therapeutic emesis · Kapha

Clears Kapha from the chest, stomach and upper respiratory tract, after several days of internal oleation.

Often used for: Kapha-dominant congestion, recurring allergic rhinitis and some chronic skin conditions.

Clears Pitta through the lower digestive tract. The most widely prescribed of the five.

Often used for: Chronic acidity (Amlapitta), inflammatory skin flares and migraine with a heat pattern.

3. Basti

Medicated enema · Vata

Clears Vata from its main seat, the colon. Charaka calls Basti "Ardha Chikitsa", half of all treatment. Given as Anuvasana (oil) or Niruha (decoction).

Often used for: Chronic low back pain, sciatica, joint stiffness and long-standing constipation.

Medicated oil or powder through the nose, which the texts call the doorway to the head. Usually a 7 to 14 day course of short sittings.

5. Raktamokshana

Controlled bloodletting · Rakta

Removes a small, measured volume of blood where vitiated dosha has localised. Leech therapy (Jalaukavacharana) is the gentler, common route, with single-use leeches.

Often used for: Gout, varicose veins and localised eczema or psoriasis patches. Not given with anaemia, bleeding disorders or blood thinners.

The 3 Phases of Genuine Panchakarma

13 to 7 days

Purvakarma: preparation

Snehana (internal and external oleation) loosens dosha lodged in the tissues, and Swedana (medicated steam) opens the channels so it moves toward the route of exit.

21 day to 30 days

Pradhanakarma: the main procedure

One or two of the five procedures, chosen by indication. Virechana or Vamana takes a single day; Basti runs as an 8, 16 or 30 day course.

33 to 7 days

Paschatkarma: recovery diet

Samsarjana Krama rebuilds digestive fire step by step, from thin gruel back to ordinary meals. Much of the benefit is kept or lost here.

Why spa packages fall short: a week of oil massage and steam is the preparation stage only. Oleation moves dosha toward the gut; if no main procedure follows, it has nowhere to go, and patients often report feeling heavy afterwards.

Chronic, recurring complaints where treatment keeps working and then keeps wearing off: long-standing digestive trouble, recurring skin flares, persistent joint and back pain, chronic sinus and headache patterns, and seasonal allergic cycles. Seasonal purification between seasons is the classical preventive use.

Deferred or modified

Pregnancy, acute fever or infection, marked frailty or advanced age, the weeks after surgery, and several ongoing medical conditions. Where cancer, kidney, liver or neurological illness is under specialist care, Ayurvedic care is supportive and sits alongside that treatment, never in place of it. What is Panchakarma?

Panchakarma is a system of five Ayurvedic purification procedures: Vamana (therapeutic emesis), Virechana (therapeutic purgation), Basti (medicated enema), Nasya (nasal administration) and Raktamokshana (controlled bloodletting). Do I need all five procedures?

Is Panchakarma painful?

Nasya, Basti and Raktamokshana feel uncomfortable rather than painful for most patients. Vamana is the demanding one, unpleasant for about an hour on the procedure morning. Virechana is generally milder than patients expect.

Is Panchakarma possible while on regular medication?

Existing medication is never stopped to begin Panchakarma. Bring a current medication list to the consultation. Some procedures are modified or ruled out on that basis, Raktamokshana with blood thinners being the clearest example.

Is Panchakarma the same as a resort detox package?

No. Resort programmes typically offer oil massage and steam, which is the preparation phase only. Classical Panchakarma adds the main purification procedure and a structured recovery diet, both under a doctor's supervision.
