What Leech Therapy Is and How It Works

Sushruta Samhita describes Raktamokshana, therapeutic bloodletting, as a Panchakarma procedure for conditions driven by vitiated Rakta (blood) and Pitta. Jalaukavacharana, leech application, is its gentlest form.

A leech draws a few millilitres of blood while it feeds, and the bite then keeps oozing for several hours because of the compounds in its saliva. Those compounds act locally:

Hirudin

A natural anticoagulant that keeps blood flowing at the bite and eases local congestion.

Bdellins and Eglins

Compounds that dampen local inflammation.

Hyaluronidase

Helps the other compounds spread into the surrounding tissue.

Anaesthetic compounds

Numb the bite area within a minute or so of attachment.

Where Leech Therapy Is Considered

Raktamokshana is considered after assessment as one part of care. It supports the main treatment and does not replace it. Conditions that need a specialist first, such as rheumatoid arthritis, high blood pressure, diabetic wounds and blood clots in the veins, stay under that specialist, and we say so at the assessment.

A Leech Therapy Sitting, Step by Step

Assessment

The doctor reviews your condition, medicines and health, and checks that nothing rules the sitting out.

Preparation

About 10 to 15 minutes: the site is cleaned with plain water and you are positioned comfortably.

Attachment

The leeches are placed on the chosen points. Attachment takes a few minutes.

Feeding

The leeches feed for roughly 20 to 45 minutes, under watch, and detach on their own.

Dressing and rest

The bites are cleaned and dressed with turmeric and honey, and you rest at the clinic before leaving with aftercare instructions.

Before and After a Sitting

The day before

A light dinner, no alcohol or smoking, good sleep and plenty of water.

On the day

A light breakfast two hours before. No coffee, tea or nicotine, and no perfumed soap, lotion or deodorant on the area; do not shave it.

The first 12 hours

The bites ooze for several hours, which is expected. Keep the dressing dry and in place, rest, and do not drive if you feel drowsy.

The next weeks

Itching around the bites for two or three days is common; do not scratch. The marks fade slowly over several weeks.

Leech Therapy Cost and Where It Is Given

Is leech therapy painful?

The bite feels like a mosquito bite or a small pinch for about 30 seconds. Within a minute the leech's saliva numbs the area, and most patients feel little during the rest of the sitting. Tell the doctor if you are anxious, and the first sitting is kept short.

How many leeches are used in one sitting?

It depends on the size of the area and the condition: a small patch of skin or a single joint needs only a few, while a larger area needs more. The doctor decides the number for you, based on your condition, constitution and response to earlier sittings.

How many sittings will I need?

Sittings are usually spaced one to two weeks apart. Acute problems often need only a few; long-standing conditions need more, sometimes with occasional maintenance sittings. The doctor sets and reviews the plan.

How long does bleeding last after leech therapy?

The bites usually ooze for about 8 to 12 hours, because the saliva keeps blood from clotting at the site. This is expected. Call the clinic if bleeding continues beyond 14 hours, or if you notice pus, fever or spreading swelling.

What are the side effects?

Expected effects are oozing for some hours, itching for two or three days, mild drowsiness, and small marks that fade over weeks. Less common are bruising, swelling, an allergic reaction to leech saliva, a local wound infection, or prolonged bleeding in someone with an undiagnosed bleeding disorder. Screening before every sitting keeps these risks low.

Are the leeches reused on other patients?

No. Each leech is used once, for one patient, and then disposed of as medical waste. This removes the risk of cross-infection between patients.

Can leech therapy transmit diseases?

Because a leech never touches more than one patient, infection cannot pass from one patient to another. The main risk is a local wound infection, which is uncommon with clean technique and good aftercare. We tell you which signs need a call to the clinic.

Who should not have leech therapy?

It is not done in pregnancy or while breastfeeding, with severe anaemia or a bleeding disorder, during an active infection, in severely immunocompromised patients, or while taking a blood thinner unless the prescribing doctor agrees. Poorly controlled diabetes, a history of keloid scars or a known leech allergy need extra caution.

Is leech therapy covered by insurance?

Most health insurance policies in India do not cover Ayurvedic procedures such as leech therapy, although some policies with AYUSH cover offer partial reimbursement. Check with your insurer; we provide detailed receipts for any claim you file.

Do I need a referral?

No. You can book a consultation directly. It helps to bring previous reports, scans, a list of your current medicines and a short medical history.

Can I drive after leech therapy?

Arrange for someone to drive you home on the day, as the saliva compounds make some patients drowsy. Most patients drive normally the next day; if the sitting was on the legs, make sure the bleeding has stopped first.

How effective is leech therapy?

It varies by condition and by person, and we do not publish percentage figures. Acute, local, congested problems such as a gout flare or an inflamed skin lesion respond sooner. Long-standing conditions such as psoriasis, osteoarthritis and varicose veins respond slowly and partially, as one part of a wider plan.

Is leech therapy better than modern medicine?

It is about appropriate use, not better or worse. Leech therapy is a local procedure with a long classical record, useful where congestion and inflammation are the problem. Emergencies, serious infections and conditions needing surgery belong with modern medicine. Any change to a prescription is made by the doctor who prescribed it.

Can pregnant women have leech therapy?
