Read this first. Rectal bleeding has many causes, and some are serious. See a physician or surgeon first, not an Ayurvedic OPD, if you are over 45 with new bleeding, if blood is mixed into the stool or the stool is black, or if bleeding comes with weight loss, a change in bowel habit, anaemia or a family history of bowel cancer. Heavy bleeding, dizziness, or a sudden hard, severely painful lump at the anus needs care the same day. Blood thinners and every other prescription continue exactly as directed.

Types of Piles, and How Ayurveda Reads Them

Doctors grade internal piles from one to four. Grade one bleeds but stays inside. Grade two comes out on straining and goes back by itself. Grade three has to be pushed back. Grade four stays out. External piles sit under the skin at the anal margin and hurt most when a clot forms in them. Grades one and two are where diet and medicines do the most; grades three and four usually need a procedure.

Ayurveda describes Arsha as fleshy growths at the Guda, the anal region, and traces them to Mandagni, weak digestion. Poorly digested food produces hard stool and gas, the person strains, and Apana Vata, the downward-moving force, is disturbed. Classical texts sort piles into Shushka Arsha, dry piles that do not bleed, and Sravi or Raktarsha, piles that bleed.

Non-bleeding piles (Shushka Arsha)
Hard, dry stool, a feeling of a lump, itching, discomfort on sitting, and pain when passing stool. A Vata and Kapha pattern. Care centres on softening the stool and warmth.
Bleeding piles (Raktarsha)
Bright red blood on the stool or the paper, burning, and often acidity alongside. A Pitta and Rakta pattern. Care centres on cooling food, and bleeding is examined before it is treated.
Prolapsing piles
A swelling that comes out on passing stool. Whether it returns by itself, needs pushing back, or stays out decides between medicines and a procedure.
Piles in pregnancy and after delivery
Common and usually temporary. Care is gentle: diet, stool softening, sitz baths and safe local applications, with your obstetrician informed.

Ayurvedic Diet for Piles

No medicine for piles works while the stool stays hard. The aim of the diet is a soft, formed stool passed once or twice a day without straining.

Eat more of
Cooked vegetables, leafy greens, gourds and Suran (elephant foot yam), the vegetable Ayurveda recommends most for piles. Whole grains, moong dal, ripe papaya, figs, soaked raisins and guava. A teaspoon of ghee with meals.

Drink
Two to three litres of water through the day, and a glass of Takra, thin buttermilk with roasted cumin and a pinch of rock salt, after lunch. Classical texts single out Takra as the daily drink for piles.

Cut down on
Chilli, pickles, fried and spicy food, red meat, maida and bakery products, excess tea and coffee, and alcohol. These either harden the stool or heat the Pitta that makes piles bleed.

Eat on time
Fixed meal times, an early light dinner, and no long fasting gaps. Irregular meals disturb Agni, and Agni is where Ayurveda says piles begin.

Home Care That Helps

Do not strain, and do not sit long
Go when the urge comes, limit toilet time to a few minutes, and leave the phone outside. A low footstool under the feet straightens the passage.

Sitz bath
Sit in a tub of plain warm water for ten to fifteen minutes, once or twice a day and after passing stool. It eases pain, spasm and itching.

Clean gently
Wash with water and pat dry. Avoid rough paper, soap and scented wipes on sore skin.

Keep moving
A daily walk and less unbroken sitting. Heavy weightlifting and straining exercises wait until the piles settle.

Be careful with home remedies
Do not apply garlic, chilli, undiluted oils or caustic pastes to piles. Use local applications only as the doctor advises.

Ayurvedic Medicines and Plants Used for Piles

These are the classical medicines Ayurveda uses, named here for information. Which of them suits you, in what form and for how long, is decided at the consultation. Self-medication with laxatives, including Ayurvedic ones, often makes piles worse.

For digestion and soft stool: Triphala, a combination of Amalaki, Bibhitaki and Haritaki; Haritaki (Terminalia chebula) alone; and Abhayarishta, a fermented preparation based on Haritaki. For swelling and healing: Triphala Guggulu and Kankayana Vati. For bleeding piles: cooling, astringent plants such as Nagakesara (Mesua ferrea) and Kutaja, used only after the bleeding has been examined. For local use: Jatyadi and Kasisadi oils, and Karanja or Nimba (neem) based oils for external piles.

Suran, elephant foot yam, deserves its own mention. Classical texts call it the foremost plant for Arsha, and it is used both as food and in medicines such as Suran Vataka.

Procedures in Ayurveda, and When Surgery Is Better

Ayurveda describes four lines of treatment for piles: medicines (Bheshaja), caustic application (Kshara Karma), cautery (Agni Karma) and surgery (Shastra Karma). In Kshara Karma a medicated alkaline paste is applied to an internal pile under direct vision, and in Ksharsutra a medicated thread is tied around it. They are para-surgical procedures, not home remedies. They are considered for selected second and third degree piles, after an examination, and they involve some days of discomfort and aftercare.

Kshara Karma and Ksharsutra are performed at our Ghorpadi and Manjri Bk, Hadapsar branches. Matra Basti, a small medicated oil enema, is used for Vata-type constipation. For large fourth degree piles, clotted external piles, heavy bleeding or piles with another anorectal condition, a surgeon's procedure is often the better route, and we say so plainly.
